Often talked about but frequently not understood, documentation by clinical staff is a critical aspect of the provision of health care.  Care Providers, State Surveyors, the Office of the Inspector General, Office of Long Term Living Field Representatives, Insurance providers, Recovery Audit Contractors, Zone Program Integrity Contractors, etc. are just some of the entities that scrutinize the documentation to determine care and financial compliance.  Added to the aspect of documentation is the current use of the electronic health/medical record to document care in the majority of facilities and the various capabilities the electronics allow, such as the cloning or cut and paste of notes.  This session will provide guidance on documentation concepts in general but also on those aspects impacted by the use of an electronic record.
