Full-Time, Day Shift

 

General Responsibilities:

Supports the Director of Nursing with the documentation of efficient and effective nursing care, while partnering with nursing team to achieve positive clinical outcomes and patient/family satisfaction.

Responsible for facilitating the implementation of the MDS care delivery model and processes, mentoring of staff, coordination and communication with other departments, including the Nursing Restorative Program.

Identifies and monitors skilled documentation protocols and assists with staff education where appropriate.

Education/Training: Current State license to practice as a Registered Nurse required

Experience: A minimum of two (2) years' experience in related nursing positions preferred.

ADA: Ability to sit, stand, lift, bend, reach, push, and pull items weighing up to 50 lbs.; visual and auditory skills.

Registered Nurse Assessment Coordinator (RNAC) 

 

Barclay Friends, a premier not-for-profit nursing and post-acute rehab community, is seeking a full-time RNAC to join the Health Services department to coordinate the completion, transmission and monitoring of the MDS assessment process in accordance with State, Federal and Barclay Friends standards. The RNAC also assesses residents, completes triggers, protocols, and care plans associated with the MDS for each resident in the skilled nursing unit. Acts as the facility appointed Infection Preventionist. Other job duties may be assigned as needed. 

This is a full-time, exempt position. The typical position hours are Monday-Friday, 7am-3:30pm. This position includes additional hours on an as-needed basis. Requires a flexible work schedule, ability to work all three shifts. 

Interested candidates must be a graduate of an accredited school of nursing and current licensure by the Commonwealth of Pennsylvania as a Registered Nurse. Minimum of two years’ experience as a professional nurse in long-term care required.  Must be computer literate and demonstrate the ability to utilize the electronic medical record and assist in daily management of associated functions and systems according to policy and procedure. Knowledge of MatrixCare preferred. Must have strong clinical assessment and leadership skills. Must have an understanding of PDPM.     Neshaminy Manor is the 360 bed sub-acute/skilled care facility owned and operated by the County of Bucks.  

    Working in a spacious office, this HIM position is primarily responsible for analysis of medical record documentation, assignment of accurate ICD-10 codes, completion of portions of the MDS and maintenance of the diagnosis list, all within our electronic charting system (ECS).  Additional job diversity of QI auditing and performing other HIM functions are also expected.   

    Teamwork is paramount within this boutique department which consists of three HIM employeesSix Unit Secretaries are an extension of the department stationed on each of the 60 bed Nursing Units.  

    SALARY RANGE:  $55,598 – 57,834+. 

    COMPENSATION: County and Union benefits equivalent to an $18,000 benefit package including pension, and medical/dental/vision.  14 paid holidays, vacation and sick time accrual.  Starting salary with increase in 6 months and a COL increase in January per Union contract.

    The Registered Nurse Assessment Coordinator (RNAC) provides for the initial assessment and periodic re-evaluation of all residents admitted to Skilled Nursing. Conduct Interdisciplinary Team Care Conference and implement and update resident Plan of Care.

     

    Registered Nurse Assessment Coordinator (RNAC) Essential Duties and Responsibilities include, but are not limited to:

    • Ensure accuracy of all sections of Multiple Data Set (MDS) coding to maximize company reimbursement consistent with the levels of treatment delivered and required for appropriate patient / resident care (CMI).
    • Ensure accuracy of clinical paperwork (assessment and accompanying documentation) to maximize company reimbursement consistent with the levels of treatment delivered and required for appropriate patient / resident care.
    • Nursing Educator: – Serve as a primary trainer to the nursing department to ensure documentation is accurate, timely, and thorough. This requires high levels of in-person interfacing with both residents and employees.
    • Nurse Management: Serves as a key member of the nursing leadership team to clinically manage the unit in order to maximize patient-centered outcomes. This includes engagement in leadership standups and influence within interdisciplinary teams.
    • Case management w/ managed care (maximize therapy and nursing services provided to residents in order to maximize patient-centered outcomes)
    • This position manages clinical and financial record keeping and dissemination of these records to appropriate payers. This includes overseeing UMR reviews and any appeals processes.
    • Take joint ownership of care conferences with social workers.

     Reporting structure:

    • Reports directly to DoN; Dotted line reporting to Director of Rehabilitation.
    • Surrogate for DoN in managing Nurse Managers
